Approximately 2% of people between the ages of 70 and 83 suffer from moderate or greater aortic regurgitation (AR) in the United States. Left untreated, this disease is progressive and fatal; however, up to 8% of patients with AR, who meet the criteria for surgical intervention, do not receive treatment.

    A dobutamine stress echo may be helpful in determining the severity of the patient’s aortic stenosis and whether the patient has any contractile reserve. If the patient’s aortic stenosis is severe, he certainly may benefit from TAVR and his EF may improve significantly. I had a similar patient 2 years ago with Class IV CHF and he is now doing extremely well and his EF normalized after TAVR.
