Preoperative imaging suggested a bicuspid aortic valve; however, intraoperative inspection revealed a rare pentacuspid aortic valve with five variably sized cusps, some exhibiting partial fusion. The final diagnosis was severe aortic regurgitation and ascending aortic aneurysm due to a pentacuspid valve anomaly.
